Complete Data Sharing Agreements for;

• 2 x CrossOrgTasks (Sending and receiving Tasks both ways)
• 2 x Cross Org Appointment (booking into Hub, booking back to Practice)
• 1 x Enterprise Appointments (Consultation Write Back) – complete on EMISNow here

Care Record Sharing One to Many – which allows the patient’s GP Practice to view the Care Record data held at your hub will not be required if using Consultation Write Back

Before you submit your requests, you will need to:

• Discuss your requirements with your local information governance (IG) officer and ask them to make any local arrangements for data sharing.
• Make sure that the data controller (practice manager, senior partner, lead GP, IG lead or Caldicott guardian) of each organisation that you want to include in a new sharing agreement is happy for the organisation to be included.

Definitions

EMIS use the following descriptions of organisations in the data sharing request forms.

Term	Definition
Requesting organisation	The organisation requesting is a sharing agreement.This organisation owns the sharing agreement, but may not be part of the sharing agreement itself (i.e. may not want to share or view data).
Sharing organisation	An organisation that shares its data with one or more other organisations.
Viewing organisation	An organisation that views data from one or more other organisations.

Cross-organisation Tasks	One to Many	Allows you to send EMIS Web Tasks to a patient’s GP practice.

Cross-organisation Appointments	One to Many	Allows practices to view and book appointments at your hub.
Consultation Write Back	EMISNow	Allows you to use the Consultation Write Back feature.

Download and complete the Data Sharing Agreements (download from links above) or from the EMISNow site.  The Data Controller required on the DSAs is usually the Practice or Business Manager at the GP Practice.  Once completed email to deliveryoperations@emishealth.com
